Potential Exam Proctor:
As a Registered Radiation Protection Technologist, RRPT, I am required
to participate in professional development activities to maintain
active registration. I need to earn 20 Registration Maintenance
Points (RM Points) during each five year renewal cycle. Pacific
Radiation Corporation has been authorized by the National Registry
of Radiation Protection Technologists Continuing Education Committee
to offer 2, 4 or 6 RM Points for successful completion of 1 to 3
Units of their Radiation Protection Technology Self Study Course.
Successful completion is measured by my taking a written examination
under closed book, closed notes conditions. This written examination
must be proctored by one of my work supervisors to meet the requirement.
I would like to ask you to serve as proctor for my examination.
This includes the following responsibilities:
1) Accept delivery of the Exam Package direct from Pacific Radiation
Corporation and maintain security over the exam before, during and
after the exam date.
2) Establish a date and time, that is mutually satisfactory to the
both of us, for taking the written exam.
3) Personally proctor the examination by remaining physically present
in the exam room while I am sitting for the examination. I am
allowed 45 minutes for a 2 RM Points Exam and 90 minutes for a
4 or 6 RM Points Exam. You are to assure that I receive no outside
assistance during the exam, and that I take the exam under closed
book, closed notes conditions using only a non-programmable scientific
calculator.
4) At the conclusion of the exam, sign and date the answer sheet
and promptly return the exam papers to Pacific Radiation, using
the postage paid envelope provided, after I have completed the
exam.
